Meredith is talking with the bartender and Derek is watching because he thinks he might have a chance with her. This how Meredith and Derek meet for the first time.

Derek says "I'm in love with you" Izzie finally gets up.

Alex and Izzie first meet. Both of them start talking and tell to each other how hard they want to became a surgeon.
